- has intentionally or knowingly failed to maintain records required to be kept by this chapter or a rule adopted under this chapter;
- has refused to allow an inspection authorized by this chapter or a rule adopted under this chapter;
- has intentionally or knowingly violated this chapter or a rule adopted under this chapter; or
- has voluntarily surrendered a registration that has not been reinstated.
